Brief Overview:          

VMCH Support for Carers Program is supporting carers across Victoria (excluding East Gippsland & Wellington LGA’S).  We want to ensure that you can continue in your caring role while maintaining your health and wellbeing.  We are responsive to your individual needs and focused on your wellbeing. We appreciate what you do daily, and how that can impact upon your health and wellbeing.  We offer special events, carer wellbeing activities, education, and referrals to long term support services if available.

Target Population

• Carers residing in Victoria, in a caring role (apart from those residing in Wellington and East Gippsland LGAs).

 Eligibility Criteria

People living in Victoria except for those residing in Wellington and East Gippsland LGA’s.

We’re here for people who care for someone with care needs including:

• Mental illness
• Dementia
• Disability
• Chronic condition or illness
• Older people with care needs
• People receiving palliative care

 Referral Pathway

Carers can self-refer by simply calling 1300 698 624 to have a chat about what’s going on for you.  You can also book a call back online and one of our team will reach out at a time you specify https://vmch.com.au/support-for-carers/carers/
